Many patients are on regular medication which can be requested without seeing your doctor. When you are on regular medication you will be given a computer printout of the list of medication. Please select which items you require by ticking each item and then returning your prescription form to the surgery.
If you do not have a repeat prescription request form please write your name, address, doctor and items required on a piece of paper.
Please indicate if you would like the prescription to be sent to a chemist of your choice. The chemists used by our patients are:-
Fetcham - Cannon Court, Boots Fetcham, Boots Bookham and The Grove Pharmacy
Leatherhead – Day Lewis , Buckley Pharmacy & Boots.
If you request your prescription through your chemist it takes us the same amount of time to process.
Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip. Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
As of March 4th, when collecting prescriptions a form of I.D. must be shown.
If someone else is is to collect your prescription, a form from reception must be completed.
Please allow four full working days for prescriptions to be processed and remember to take weekends and bank holidays into account.
Extensive exemption and remission arrangements protect those likely to have difficulty in paying charges (NHS prescription and dental charges, optical and hospital travel costs). The NHS prescription charge is a flat-rate amount which successive Governments have thought it reasonable to charge for those who can afford to pay for their medicines. Prescription prepayment certificates (PPCs) offer real savings for people who need extensive medication.
These charges apply in England only. In Northern Ireland, Scotland and Wales prescriptions are free of charge.
If you will have to pay for four or more prescription items in three months, or more than 15 items in 12 months, you may find it cheaper to buy a PPC.
There is further information about prescription exemptions and fees on the NHS website
Q. When should I put in my repeat request?
A. When you have one week’s supply of tablets left. If you are going away you can put in your requests earlier than usual — just put a note on it so say why you are requesting your prescriptions early.
Q Why don’t you take repeat prescriptions over the telephone?
A. The surgery has over 6000 patients and we issue prescriptions for 6300 items every month. If every patient telephoned us we would not be able to get any other work done and the telephone would be constantly engaged. Once you have used the computer print out you will see how easy it is.
Q. I have lost my computer print out. What should I do?
A. No problem! Just drop a piece of paper into the surgery with your name, address, your doctor’s name and the medication you need. When you collect your prescription there will be a new computer list for you to keep.
Q. Will you post my repeat prescription to me?
A. Yes of course. We will need a stamped addressed envelope. If you do not provide us with a SAE, we will keep your prescription at the surgery for you to collect.
Q. Can you send my prescription to any chemist?
If you wish your paper prescription to to sent to a chemist, only the following ones have a collection service from the surgeries: -
For Fetcham - Cannon Court, Boots Fetcham, Boots Bookham and The Grove Pharmacy
For Leatherhead – Day Lewis , Buckley Pharmacy & Boots.
If you wish your paper prescription sent to any other pharmacy, we are happy to do so providing you give us a stamped, addressed, envelope.
If you sign up for Electronic Prescription Service (EPS) your prescription will go to your nominated chemist.
