New Patient Registrationcannonside-58ky-oval

We welcome new patients to our practice.

When you register with one of our doctors you will be required to put your new details onto your medical card issued to you by the health authority when you registered with your previous GP. If you have mislaid your medical card then you can fill in a New Patient Registration Form available at the practice or as a download from the above "Registering Online" tab

You will be asked to return and make an appointment with a practice nurse for a health check within one month unless you need repeat medication (in which case an earlier appointment should be made with the doctor to have the medication authorised)

kingston-rd-59ky-ovalThe health check involves an examination and questions will be asked about your current and past health in order that we may identify any preventable medical problems. You will also be required to provide a urine specimen.

You may see anyone of our doctors anytime, not just the doctor you are registered with.

Medical treatment is available from the date of registration. Please contact reception for further information.
